Patient Terms of Use

1. DEFINITIONS

“Company” means Traumalis, a Georgia LLC, and its affiliates, officers, directors, employees, and agents.

“Service” means the software-as-a-service platform provided by Company.

“Clinician” means licensed healthcare professionals authorized by Customer to access and use the Service.

“Customer” means the organization providing treatment to patient and employing Clinician.

“Patient” means individuals receiving care from Customer’s Clinicians using Service.

“PHI” means Protected Health Information as defined under HIPAA.

2. IMPORTANT NOTICE

This Service is provided to Patient (“you”) through your healthcare provider. You do not have a direct account or business relationship with Traumalis, LLC (“Company”).

3. CLINICAL SUPERVISION REQUIRED

3.1 Healthcare Provider Responsibility: Your healthcare provider is solely responsible for your medical care and any diagnostic impressions or treatment recommendations that result from your use of this Service.

3.2 No Direct Medical Service: This Service does not provide medical advice or clinical diagnosis, or treatment.

4. EMERGENCY DISCLAIMER

THIS SERVICE IS NOT FOR EMERGENCIES: Do NOT use this Service if you are currently experiencing a mental health crisis. In the event you experience a medical emergency while using this Service, immediately discontinue use of the Service. Report to your healthcare provider, call 911, or go to your nearest emergency room.

5. LIMITATIONS

5.1 Support Tool Only: The Service assists your healthcare provider but does not replace their clinical judgment or therapeutic intervention.

5.2 No Guarantee: No medical outcomes are guaranteed from use of this Service.

5.3 Provider Relationship: Your healthcare provider-patient relationship governs all medical decisions.

6. LIABILITY

Your healthcare provider, not Company, is responsible for your medical care and any claims related to treatment decisions or outcomes.

7. PRIVACY

Your privacy is protected through your healthcare provider’s privacy practices and policies. Company processes information only as necessary to provide the Service.

8. QUESTIONS AND CONCERNS

Direct all medical questions and concerns to your healthcare provider, not to Company.

9. ADDITIONAL POLICIES

Your use of the service is also governed by our:

These policies are incorporated herein by reference and form part of these Terms of Use.
